Welcome to Mary Our Queen Catholic School! To begin the application process for enrollment in grades K-8 for the 2022-23 school year, please complete and submit the requested information below or call the school office at 402-333-8663.
The New Family Application is the first step in the enrollment process.

Listed below are the required documents for enrollment. Please Click on each Form to download.

- Archdiocese General Information Form (1 per student)
- Student Information/Emergency Contact Form Page 1
- Student Information/Emergency Contact Form Page 2
- Student Health Form (1 per student)
- Dismissal Instructions (1 per family)
- Family Handbook and Technology Use Policy Agreement (1 per family)
- Authorization for Release of Records if student is transferring from another school
Please also include the following documents with your enrollment documents:
- Copy of each student’s birth certificate
- Copy of each student’s baptismal certificate (if applicable)
- Copy of each student’s immunization record
- $250 New Family Registration Fee (This fee is non-refundable. Once enrolled, this fee is applied to the 2022-23 tuition.)
Once enrolled, families will receive an email from FACTS Management Company with information to set up their tuition payment account.
